麻酔薬と誘発電位に関する研究研究テーマ一覧へ
手術中に神経機能のモニタリングの一つとして誘発電位の測定が脊椎手術、脳神経外科手術や大血管手術の際に行われています、その一方で様々な麻酔薬が誘発電位、特に運動誘発電位(Motor-evoked
potential; MEP)を減衰させることが知られています。吸入麻酔薬は静脈麻酔薬に比べ誘発電位に及ぼす影響が大きいことはよく知られていますが、静脈麻酔薬や麻薬性鎮痛薬がどの程度運動誘発電位に影響を及ぼすのか、実際のところわからないことが多いです。教科書に載っている記述の中には非常に古い研究結果が踏襲されているものもあり、改めて検証しなければいけないものもあります。
多くの高次医療機関では麻酔科のマンパワーの問題から、脊髄機能モニタリングは主に臨床工学士(ME)の手により行われていることが多いですが、新潟大学麻酔科では、脊椎外科、心臓血管外科における脊髄機能モニタリングを臨床工学士と共同して麻酔科が担っています。臨床ばかりでなく、誘発電位と麻酔薬の関係についての基礎的研究においても古くから研究を行ってきました。
当研究室では主に脊椎外科の協力の下、様々な静脈麻酔薬の誘発電位に及ぼす影響(特に脊髄における作用)についての臨床研究を行っています。その中には教科書を書き換えるような内容の研究も含まれます。これらの臨床研究を通じて、麻酔薬の作用メカニズム解明、あるいは術中神経モニタリングの精度向上に繋がるものと考えています。
